Premium Eye Centers may use SMS communications to contact and communicate with patients to remind them of upcoming appointments and medical procedures, confirm and schedule appointments, assist with answering questions, and provide reminders regarding insurance, procedure preparation, anesthesia, and other requirements.

1. Messaging Frequency

We may send SMS messages regarding appointment confirmations, reminders, updates, and educational/promotional materials.
  • You will receive up to 5 messages per week.
  • Message frequency may vary depending on your engagement with our services.

2. Cost and Fees

  • Message and data rates may apply. Depending on your text plan, you may be charged by your carrier.

3. Opt-in Process

By subscribing to our SMS alerts, you confirm that you agree to receive messages from Premium Eye Centers. The opt-in process is as follows:
  • Users can opt in by filling out a form on our website.
  • START/JOIN Command: Patients may Opt-IN to getting SMS communications by sending an SMS to (305) 222-7082 indicating their desire to receive SMS communications from Premium Eye Centers by sending the keyword JOIN.

4. Opt-out and Cancellation

Patients may Opt-Out of SMS communications at any time.
  • STOP Command: At any time, you can text STOP. This will prevent you from receiving any future texts from Premium Eye Centers Notifications.
  • After sending STOP, you will no longer receive any texts from Premium Eye Centers.

  • Participating Carriers: AT&T, T-Mobile®, Verizon Wireless, Sprint, Boost, U.S. Cellular, Google Voice, Cellular One, MetroPCS, Cellcom, Cellular South, Carolina West Wireless, Interop, ClearSky, nTelos, and Virgin Mobile.
  • Carrier Liability: The Carriers do not guarantee that alerts will be delivered and will not be liable for delayed or undelivered messages.
  • We do not guarantee uninterrupted delivery of SMS messages. Factors such as network availability and carrier issues may affect message transmission.

8. Changes to Terms and Conditions

Premium Eye Centers reserves the right to modify these Terms and Conditions at any time.

  • Continued use of our SMS services after modifications constitutes acceptance of the revised terms.
